The American Lung Association of the Central StatesĀ® is dedicated to protecting the privacy of our online visitors. You may choose to supply personal information in eight different areas on this site:

  • “Site Membership” Registration:

When you utilize the American Lung Association of the Central States web site, http://www.breathehealthy.org, you may be asked to provide the following information: your name, e-mail address, zip code, age, member name and password.

In addition, you may have the option of letting the American Lung Association of the Central States contact you in the future. This form also requests additional information, including: your address, household income, education, marital status and occupation. This information helps us define the demographic make-up or our visitors online.

  • “Donating” to the American Lung Association of the Central States:

When you donate to the American Lung Association of the Central States online you submit highly sensitive credit card information, contact information, and acknowledge whether you would like to be contacted by ALACS via email in the future. We recognize the importance of respecting the integrity of this material and limit access only to personnel authorized to process your request.

You may also choose to make a gift donation. When you make a gift donation, the amount of your gift is shown only on your receipt. The person or family you designate will receive an acknowledgment card in your name. We collect this prospect information in order to honor your request to send an acknowledgement card. Prospect information is forwarded to the local lung association associated with the prospect, but is not included on any Direct Mail listings for the American Lung Association. E-communications from the American Lung Association are only issued through the direct consent of each unique visitor.

Online donors are also offered the option of choosing to let the American Lung Association contact them via email in the future.

  • The “ALA Survey”:

During any number of site promotions, visitors may be encouraged to fill out an online survey for the American Lung Association of the Central States. This survey asks for contact information and user profile information like sex, age, education, internet connection, whether a family member has lung disease, area of lung health interest and occupation. We also ask if the visitor has ever donated online, and if so, how much, and whether the visitor has ever done any volunteering in the past year. These questions help us define the needs of our users.

We also ask if the visitor would allow the American Lung Association of the Central States to contact them in the future via email.

  • Online “Volunteerism” Submissions:

The online form in the Volunteer section of the site requests state, name, mailing address, email address, and a place where you can describe your area of interest. The information that you provide here is forwarded to the local lung association in your area, who will then contact you about volunteerism opportunities best suited to your interests.

  • Signing the “Wall of Remembrance” or the “Living with Lung Disease Wall”

When you choose to sign either one of our two ‘Walls,’ you are posting a user name, email address, and message that you define on a public forum.

  • Registering for “Freedom From SmokingĀ® Online:”

Registrants to our free online smoking cessation program fill out a form that supplies required and unrequired information. The required information enables the American Lung Association to better support your smoking cessation effort, allowing you to opt-in to further email contact by the American Lung Association. The unrequired information mirrors questions asked in our “off-line” smoking cessation program and offers valuable demographic information on Freedom From SmokingĀ® online users.

The American Lung Association reserves the right to offer an e-communication option to our web visitors.

You understand and agree that the American Lung Association of the Central States may disclose to third parties your zip code, gender and/or age, but only in the form of aggregated information.

The American Lung Association of the Central States reserves the right to offer you third party services and products based on the preferences that you identify during the registration process and based on your subsequent preferences; such offers may be provided to you by the American Lung Association of the Central States.

The American Lung Association of the Central States reserves the right to terminate your account if it learns that you have provided false or misleading registration information.
